Does this mean all the faction work has been done maxing the TrueSpirit faction for the final kill?

If all he has is the tear than you are way off of completing the epic.

Anyone have any info on how much work/time is involved in factioning for this portion of epic?

Depends on how you do it. If you go through whole quest, its pretty "short" (not really) requires raid-ish sized group to kill some mobs for quest work.

Or, you can grind it (easily soloable) but takes a while, and I believe corpse banking makes it faster, but still long.
