Default Medallion of the Obulus Multiquest
The Multiquest for the medallion pieces is broken. It ate my pained soul + Rotting Skeleton + Burnished Stave Medallion pieces. I've also heard of this happening to other people.

This seems to have started occurring after epics were implemented and Slixin Klex became necessary for mages to kill.
